Ikegami/Stephan, JAPANESE BOOKBINDING (Weatherhill) will give you the
details on the traditional Japanese format developed over the last several
centuries. There are, of course, ways to vary it but the basic construction
is very sound and beautifully simple.

Keith Smith's discussion is a good one except for the omission of the inner
binding step, a "corner" which began to be cut even by Japanese binders a
century or so ago as competition with machine binding and the demands of
the mass market grew.
