Definitely ask the book owner(s) what they would like to do.

You outlined all of your options, but if you are rebinding, consider adding
folios recto and verso onto which the inclusions can be tipped.

The practice in French bookbinding is to add these folios to the original
text block, where any additional materials relating to the book can be added
- recto for the front of the book, verso for the back matter. Usually this is
for binder's notes, sale or transfer of the binding, original dust jacket and
such. Your inclusionary items would be divided according to the order in the
church records.
