It seems that the reason for the paper twist step is being
overlooked.  Structurally, the paper twist is to hold the text pages of a
book firmly in place so trimming of head, tail and spine can be carried
out. This is more obvious with a printed book whose pages have been
carefully aligned and further shifting is undesirable. Trimming can't take
place with the outer thread in place. The outer thread is the cover
attachment mechanism.  When the outer thread breaks or the cover wears out,
the paper twists still hold the text in order so recovering is easy.
