I say a structure at the Book Arts collection at the University of
Washington that had signatures sewn to a concertina spine, then the
concertina spine was side sewn in a Japanese style.

With this in mind, you could sew two concertinas together (mountain to
mountain), glue your thick paper to one side of the outer concertina and
side sew the inner concertina.  The hinge action would take place between
the two concertinas.

I've seen the thick paper scored before sewing but it never opens well or
lays flat.
