Thanks, Peter. One you didn't list there that specifically addresses Ben's
questions is the introductory essay to the _Book Arts in the USA_ exhibition
catalog (1990). You posted the entire text to book_arts-l in 1994, and it is in
the archive at

Since then it has been added to the Center for Book Arts website in its original
format, in  English and in French, and is illustrated with examples of
traditional binding, letterpress printing and typography, photocopied, computer
generated, and sculptural bookworks, unique diaristic book art, etc.  If you
click on "What is Book Art" at the CBA home page, you get there, or go directly
