Barton Lidicé Benes did many amazing book objects in the early 1970's. His Censored Book (1974) has been copied by many other artists, some of whom probably never saw the original. I've seen at least 3 other artists' works with the same concept and title in various exhibitions. You can see a picture of the original, which is in the collection of the Center for Book Arts, at
