I'm sure you know about this one already but just in case you don't The
International Institute of Social History has, or at least had a 17th
century vade mecum in its collection. It is described at
http://www.iisg.nl/friends/newsletter-5.pdf (page 3) and there is a
photograph at  http://www.neha.nl/collections/aanwinsten2001.php. While
this is not specifically a medical vade mecum it may be of some help.
